Revision Surgery
Am I Eligible for Revision Surgery?
Revision surgery may be needed if the person is unable to establish healthy eating habits, is unable to achieve adequate weight loss or if the person is observed to gain weight again for 18 months. Revision surgery is a method used when a surgeon who has not gained experience or has not achieved sufficient weight loss as a result of technical deficiencies. It can be applied to all patients who have undergone obesity surgery by any method before and gain weight again or who cannot lose enough weight. However, the method to be applied to each patient is different. Therefore, not all methods can be applied to every patient. Again, all of these procedures laparoscopically.
Pre-op Assessment
Treatment should be planned according to the time and size of the problems and problems that you experience after your obesity surgery. Of course, not every patient with a problem is offered an immediate revision. First of all, the underlying causes of this problem, anatomical and functional and of course, the patient’s factors should be investigated and the surgical correction should be checked whether there are possible. The biggest reason for weight gain, especially after Gastric Bypass, is that patients are unable to give up their sugar eating habits. This is usually caused by Dumping syndrome. When these patients are examined, posh deformations are common. Eating modifications do not provide much benefit in these types of patients. Thinking about surgical correction at the forefront is essential for permanent control of the problem.
First of all, detailed information about the patient’s previous surgery should be obtained. First there must be epicrisis about surgery and diseases. Endoscopy should preferably be performed by the physician who will perform the surgery. In addition to endoscopy, these patients may also need barium graphies, medicated abdominal tomography, or medicated magnetic resonance imaging methods to better understand the anatomical changes caused by the previous surgery.
Band revision
The gastric band is artificial silicone prosthesis. It can be described as a “volume-limiting surgery” whose function is to restrict only solid food intake. Regardless of the problems that develop, it is best to remove the gastric band completely. In some cases, in the same surgery where the gastric band is removed, a revision may be applied to another surgery. However, this is a case that needs to be carefully evaluated in every patient. This choice can lead to unforeseen very severe complications, fatal situations, very long hospital stays, multiple surgeries and heavy economic costs, considering that it is only easy, the work will be finished in one operation, it will be more economical and it will be a shorter treatment process.
Bypass Revision
It is a technique that has been applied most in the world and is now starting to be stated that almost no prior examination is even needed. The symptoms of reflux can be controlled by simply shrinking the stomach and transmitting saffron away from the stomach. In Gastric bypass surgery, the most common area of constriction is the part in the middle of the stomach. Here there is a natural curve of the stomach. Sometimes, when the stomach is shrinking, it is necessary to pass close to the area. This type of constriction usually develops months after surgery with the formation of scar tissue and clinically creates difficulty eating. In this case, the priority is “Endoscopic Balloon dilation”. The area that is constricted without surgery is observed with endoscopy and there is a balloon that is inflated with a few minutes of dilation process. In most patients, the results are laughable. Several sessions may be required.
Sleeve revision
If the patient has lost less than 50% of his or her excess weight within 18 months of the first bariatric surgery; If complications develop in the long or late period due to the first operation; If you have gained weight again after losing weight, the first operation is considered a failure and revision surgery is on the agenda. Many bariatric surgeons prefer to turn a failed restrictive surgery into a malapsorbtive or combined method. In addition to being a gastric constricting operation, the tube also acts on metabolism in metabolic terms as it increases GLP-1 and peptide YY levels after surgery. This feature distinguishes him from other restrictive obesity surgeries. Revision surgical procedures can be performed after restrictive operations that have failed due to these metabolic effects.
